Another type of injection is a transforaminal steroid injection. For months doctors told her the knife-like pains were in her head, but six months later, Bertelli was diagnosed with arachnoiditis , an incurable condition that can be associated with epidural steroid injections.
The preservatives in steroids can be "toxic" to spinal nerves. Epidural injections for anesthesia in childbirth are also associated with arachnoiditis when a doctor inadvertently punctures the dura and a blood patch is used to close up the hole.
Dura punctures happen about 4 percent of the time in childbirth. Many doctors tell patients that the worst outcome is that epidural steroid injections "may not work," rather than spelling out the risks.
Anesthesiologist Baker argues that overall, the risks of accidental dural puncture are small, according to medical literature -- about 0. In obstetrical patients using a "blind" without an x-ray technique, the risk rises to 1 percent. In one Japanese study of patients, none of the 89 patients who received steroids in their spinal fluid because of dural puncture developed arachnoiditis.
Baker estimates at most, the odds of getting arachnoiditis are about 1 in 50, Some studies have shown an association between arachnoiditis and exposure to herpes or cytomegalovirus , according to Bertelli.
Two-thirds who develop the condition end up losing bowel or bladder function and some end up bedridden, requiring powerful opiates. Such was the case with Matt Clark, a year-old father of two from Tennessee. Just hours after receiving an epidural steroid injections for back pain in , parts of his intestines had dropped into his underwear because the epidural had relaxed the muscles in his gut.
Now, felled with arachnoiditis, he is disabled, in permanent pain with stabbing nerve pain in his feet and limbs and the sensation of invisible insects crawling on his skin. I don't want to hurt my family but I truly would pick death over this disease. He has even considered suicide.
Clark compares the overuse of spinal injections to the lobotomies of an earlier century. Everyone thinks they are wonderful, but they are not. I am going to have this for the rest of my life. Bertelli, whose condition has stabilized with medications, said she feels the same way. She is still in pain and has spent thousands of dollars on treatments. I am living with the pain, but it hits you and you try not to cry and maintain a smile -- it's important with little kids. I had no numbing effect and,in fact, everything has hurt much worse ever since.
I walked into the building just fine on my own and my husband had to practically carry me out. The spine is such a sensitive zone. I just think its so easy for mistakes to be made.
Diagnosis is difficult in that area. I intend to see what my regular doctor and my rheumatologist say about this experience as it was under both their recommendations that I go have this done. I trust my regular doctor but I'm not sure he made the right call here with the place they sent me. The procedure itself was horrifically painful and I was not prepared for that. They told me that usually it is not uncomfortable for people.

It was appreciated, from the beginning, that myelography was much more informative than epidurography. The medical profession has not yet appeared to recognize that the epidural and subarachnoid spaces are very different in their response to foreign body substances.
The epidural space is highly resistant to insult while the subarachnoid space is not. It has also not been well appreciated that when potentially neurotoxic substances are introduced into the epidural space for diagnostic or therapeutic purposes great care is required to assure that these substances are not actually placed into the more fragile and pristine subarachnoid space.
Clinically significant adhesive arachnoiditis represents a serious potential disaster for any unsuspecting patient in whom unsafe materials are injected.
Despite the seriousness of this problem and good scientific documentation actually available in the medical literature on this subject there does not yet exist a general level of awareness among the medical profession regarding this potential patient disaster.
Given this fact it is unlikely that many patients have had, prior to epidural steroid injection, legitimate informed consent. A summary illustration of the process of adhesive arachnoiditis is shown above to demonstrate its separation from the epidural space.. It seems that only when physicians, or their patients , have firsthand experience with this disease do they come to any real understanding of it.
